Introduction:

Community Care Program (CCP) is supported and administered by Illinois Department on Aging (IDoA). It provides in-home and community-based services to eligible Illinois seniors aged sixty (60) or over.

Eligibility Requirement:

  • Sixty (60) years of age of older,
  • Reside in Chicago areas with zip codes 60613, 60614, 60618, 60625, 60626, 60630, 60631, 60640, 60645, 60646, 60647, 60656, 60657, 60659, 60660.
  • United States citizen or Legal Alien.
  • With Medicaid or Medicare.
  • Limited non-exempt assets.
  • Chinese, Vietnamese, Filipino, Korean, and Japanese are welcomed.

Service Tasks:

If eligible, client is provided with a qualified homemaker who will perform the following services:

  • Assisting with personal care (grooming, bathing, dressing),
  • Preparing and serving meals,
  • Performing routine household tasks (vacuuming, washing dishes, mopping floors, doing laundry……),
  • Doing shopping and essential errands.
